Before You Come
Fill Out Your Forms (Required)
Complete your forms online before your appointment. If you’re using insurance, enter your plan details so we can verify benefits one week in advance. If your insurance isn’t confirmed by your visit, full payment will be due at the time of service.
Confirm Your Appointment
Add it to your calendar, reply “Yes” to confirm by text or email, and arrive 10–15 minutes early.
Bring Your ID and Insurance
Bring a photo ID and any dental or medical insurance cards. Check-in is quick on our tablet; you’ll snap photos of your cards.

Welcome and Goals
Tell us what matters most to you.

Complete Oral Health Exam
We check teeth, bite, and jaw, look for cavities or wear, and screen for underlying concerns.

Gum Health Check and Perio Charting
Using gentle probing, we assess your gum tissue for signs of inflammation or periodontal disease and record measurements to track gum health over time.

Digital X-rays and Panoramic Scan
(only if clinically appropriate)
Safe, low-radiation images help us see what the eye can’t — like bone levels, infections, or decay beneath the surface.

Microscope Biofilm Education
We place a tiny plaque sample on a slide and show you live bacteria under a clinical microscope so you can understand gum health and choose the best home-care routine. Education-only; diagnosis comes from your exam and X-rays.

Your Simple Plan
We review findings and agree on next steps; same-day cleaning when appropriate and available (if gum therapy is needed, we’ll outline that first).
