To help us prepare and keep your visit running smoothly, please complete the steps below:

Please complete your forms before your appointment:

If you’re using insurance, enter your insurance details on the form. We verify insurance 1 week before your visit. If not verified in time, full payment will be due at your appointment.

✔ Step 2: Confirm Your Appointment

• Add your appointment to your calendar

• Reply “Yes” to your text reminder when it arrives

• Arrive a few minutes early

✔ Step 3: Bring Your ID and Insurance Card

When you arrive, you’ll check in using our tablet.

Please bring:

• A valid photo ID

• Your insurance card (front and back)

You’ll take photos of these yourself during check-in.

What You’ll Experience at Your First Visit

Your first visit is about more than just checking teeth — it’s a chance for us to understand your complete oral health and build a care plan just for you. Here’s what’s included:

  • A complete oral health exam

    We’ll evaluate your teeth, bite, and jaw, check for cavities or wear, and look for signs of any underlying concerns.

  • Gum health check and perio charting

    Using gentle probing, we assess your gum tissue for signs of inflammation or periodontal disease and record measurements to track gum health over time.

  • Digital X-rays Panoramic Scan

    Safe, low-radiation images help us see what the eye can’t — like bone levels, infections, or decay beneath the surface.

  • Oral Bacteria Testing

    (Required $108)

    We use a microscope or saliva-based sample to identify bacteria that may be contributing to gum disease, inflammation, or other systemic risks.

    This testing is essential for us to design a personalized treatment plan based on your current oral health.

    The cost is $108 and is not billable to insurance.

  • A personalized plan tailored to your goals and health needs

    Based on what we find, we’ll create a treatment plan that supports your long-term health, comfort, and confidence — whether you’re here for a healthy mouth or a smile makeover.
